An employee was bucking a fir log when the tension released, launching the log into the air. The log landed on the employee, resulting in two fractured vertebrae in the neck, one back vertebra fracture, five broken ribs, a broken clavicle, a punctured and collapsed lung on one side, a partial collapsed lung on the other side, multiple contusions and scrapes, and a chipped tooth.

On or about August 4, 2016, an employee was transferring a tray of mail to the mail truck when the carrier stepped off a curb, felt a snap in the left ankle, dropped the tray of mail, and fell to the ground. The employee's ankle fractured in three places.

An employee was spotting the loading of a skip hoist onto a semi trailer. The employee attempted to adjust one of four legs that support the motor pack for the skip hoist, which is a separate piece from the skip hoist. The motor pack weighed 1,850 pounds. The employee was loosening a clamp on one of the motor pack legs when the leg collapsed, causing the motor pack to fall on the employee, who suffered broken vertebrae.
